Had I the $, I'd jump in the car (I don't have) and hit the road for a few years to photograph these places, one sick name at a time... I hope someone with the resources and necessary sensitivity does just that!

Stan, this might also make a great collaborative project between a group of photographers, especially ones who live in those areas of the country. I think it would be interesting to have several different voices / views as part of one big project. I'm looking at the map in the state were I live (Texas) and it might fit in with another project I'm planning, a re-photographing of the places in Texas that Shore photographed for Uncommon Places.
